Chemical Burns: Causes, Symptoms, and First Aid

What is a chemical burn?
A chemical burn is tissue damage caused by contact with a corrosive substance: an acid, a base, or an oxidizer. It can affect the skin, eyes, airways, or digestive tract. In the United States, chemical burns are a less common but serious type of burn injury, occurring at home, at school, and in the workplace. Unlike thermal burns, their severity can continue to worsen for hours after the initial contact, which makes them a medical emergency that is often underestimated.
Causes and risk factors
Chemical burns most often result from accidental contact with household or industrial products. Common causes include:
- Strong acids (hydrochloric acid, sulfuric acid) found in some household and industrial products
- Caustic bases (lye/sodium hydroxide, ammonia, quicklime) used in home improvement or cleaning
- Powerful oxidizers (concentrated bleach, hydrogen peroxide)
- Accidental ingestion of corrosive products, especially in children
Risk factors include working in a lab or chemical industry setting, lack of personal protective equipment, and improper storage of hazardous products at home.
Symptoms
Clinical signs vary depending on the product, its concentration, and exposure time. The most common symptoms are:
- Intense pain, often immediate, at the contact site
- Redness and local swelling
- Blisters in the case of a deeper burn
- Whitish, grayish, or blackened skin (tissue death)
- With eye contact: tearing, light sensitivity, blurred vision
- With inhalation: coughing, difficulty breathing, a burning sensation in the throat
- Nausea or vomiting if ingested
Diagnosis
Diagnosis is primarily clinical, based on the patient’s history (type of product, exposure time) and physical examination of the injury. Burn depth is assessed using standard degree classifications (superficial to deep). In cases of ingestion, an upper endoscopy is performed urgently to assess damage to the mucous membranes. For eye burns, a slit-lamp exam is essential. Blood tests may be ordered in cases of systemic exposure.
Treatment
Treatment depends on the location and severity of the burn.
- Thorough rinsing with water: the most critical step, to be done immediately for at least 20 to 30 minutes with lukewarm running water, without attempting to neutralize the chemical
- Local wound care: appropriate dressings, healing ointments (petroleum gauze, hydrocolloid dressings) for superficial burns
- Pain relief: acetaminophen or anti-inflammatories, or opioids for severe pain
- Eye care: lubricating eye drops, topical steroids for severe eye burns
- Surgery: skin grafting for deep or extensive burns
- If ingested: hospitalization, fasting, endoscopic or surgical care depending on the extent of injury
Prevention
Preventing chemical burns relies on a few simple but essential measures:
- Wear personal protective equipment (gloves, goggles, apron) when handling chemicals
- Store corrosive products in their original containers, out of children’s reach, in locked cabinets
- Carefully read safety data sheets (SDS) for the products you use
- Never mix household products (especially bleach and ammonia)
- Ventilate rooms when using volatile products
- Train exposed staff on emergency procedures

Seek emergency care if the burn affects the face, eyes, hands, feet, or genitals, if it covers a large surface area, or if a product was ingested or inhaled. Any deep burn (white or blackened appearance), respiratory distress, or signs of shock (paleness, feeling faint) requires an immediate call to 911.
