Autoimmunity: Causes, Symptoms, and Diagnosis

Overview
Autoimmunity refers to a malfunction of the immune system in which it mistakenly attacks the body’s own tissues and organs. In the United States, more than 23.5 million Americans are estimated to live with an autoimmune disease, with a strong female predominance. There are more than 80 distinct autoimmune diseases, including rheumatoid arthritis, systemic lupus erythematosus, multiple sclerosis, and type 1 diabetes. Together they represent a major public health issue because of their chronic nature and their impact on quality of life.
Causes and risk factors
The origin of autoimmune diseases is multifactorial. Key identified mechanisms include:
- Genetic predisposition: certain HLA (human leukocyte antigen) genes increase susceptibility, and autoimmune diseases tend to run in families
- Hormonal factors: estrogen imbalance partly explains the female predominance
- Viral or bacterial infections: molecular mimicry can trigger a cross-reactive immune response
- Environmental factors: exposure to pesticides, sun (UV), smoking, and chronic stress
- Gut dysbiosis: disruption of the gut microbiome, contributing to a breakdown in immune tolerance
Symptoms
Clinical signs vary by the targeted organ, but some are common across many autoimmune diseases:
- Unexplained chronic fatigue, often disabling and one of the most common signs
- Diffuse joint or muscle pain (arthralgia, myalgia)
- Persistent or recurring unexplained fever
- Characteristic skin rashes, such as the lupus “butterfly” rash across the cheeks and nose
- Organ inflammation: kidney, liver, lung, or neurological involvement
- Digestive issues, or dry eyes and mouth (Sjogren’s syndrome)
- Unintentional weight loss and sleep disturbances
Symptoms often come and go, flaring up severely at times and easing into periods of remission. This variability can delay diagnosis.
Diagnosis
Diagnosis relies on a multidisciplinary approach:
- A thorough clinical exam guided by symptoms and family history
- Blood work: complete blood count (CBC), CRP, ESR, antinuclear antibodies (ANA), ANCA, anti-dsDNA antibodies, and complement levels (C3, C4)
- Medical imaging: ultrasound or MRI, depending on the organ involved
- Tissue biopsy in some cases (kidney, skin, liver) to confirm the diagnosis
The diagnostic journey can take several months given the complexity and variability of clinical presentations.
Treatments
Treatment aims to control inflammation, protect organs, and maintain quality of life:
- Immunosuppressants: corticosteroids (prednisone, methylprednisolone) as first-line treatment to reduce acute inflammation
- Long-term disease-modifying drugs: methotrexate, azathioprine, and hydroxychloroquine (Plaquenil) for chronic forms
- Biologic therapies (targeted treatments): anti-TNF, anti-IL6, and anti-CD20 (rituximab) for disease resistant to standard treatment
- Symptomatic treatments: pain relievers, NSAIDs, and vitamin D and calcium supplementation
- Physical therapy and rehabilitation to preserve joint mobility
- Regular follow-up with a specialist (rheumatologist, internist, or neurologist) to adjust treatment
Because these treatments suppress immune activity, many people need ongoing care, sometimes for the rest of their lives.
Prevention
Autoimmune diseases cannot be entirely prevented because of their genetic component, but some measures can reduce the risk of onset or flare-ups:
- Quitting smoking: a recognized aggravating factor, especially in rheumatoid arthritis
- Stress management: relaxation techniques, meditation, and regular appropriate exercise
- A balanced diet rich in antioxidants and low in ultra-processed foods, supporting a healthy microbiome
- Avoiding endocrine disruptors and environmental toxins
- Regular medical monitoring if you have a family history of autoimmune disease
- Appropriate sun protection, especially for lupus
When should you see a doctor?
See a doctor promptly for unexplained intense fatigue, persistent joint pain, an unusual skin rash, or prolonged fever. Early treatment is essential to limit organ damage. Severe kidney, neurological, or cardiopulmonary involvement requires immediate emergency care.
