Cervical Osteoarthritis (Cervical Spondylosis): Symptoms and Treatment

Overview
Cervical osteoarthritis, also called cervical spondylosis, is a degenerative condition affecting the vertebrae of the neck (cervical spine). It involves the gradual wear of the cartilage and intervertebral discs between C1 and C7. It’s extremely common in the United States, affecting more than 85% of people over age 60, according to the American Academy of Orthopaedic Surgeons (AAOS), though it can start as early as your 40s. It’s one of the most common causes of chronic neck pain and a major focus area in rheumatology and orthopedics.
Causes and risk factors
Cervical spondylosis results from progressive breakdown of joint cartilage through several mechanisms. Main identified risk factors include:
- Natural aging of the joints, the leading cause
- Repetitive strain from certain jobs (screen work, heavy lifting)
- Neck injuries (whiplash, accidents)
- Excess weight, which increases pressure on the spine
- Congenital abnormalities or spinal alignment issues
- A sedentary lifestyle and lack of regular exercise
- Genetic predisposition in some early-onset forms
Symptoms
Clinical signs of cervical osteoarthritis vary by stage. The most common are:
- Neck pain, often the first sign
- Neck stiffness, especially in the morning, limiting range of motion
- Pain radiating to the shoulders, arms, or head (cervical radiculopathy)
- Headaches (cervicogenic headaches) at the base of the skull
- Dizziness and tinnitus when the upper vertebrae are involved
- In severe cases, spinal cord compression with sensory or motor problems in the limbs
Diagnosis
The doctor makes the diagnosis based on a clinical exam: assessing pain, neck mobility, and neurological function. Additional tests include:
- A cervical spine X-ray (front and side views), the first-line test showing disc space narrowing and bone spurs (osteophytes)
- A CT scan or cervical MRI if nerve or spinal cord compression is suspected
- An electromyogram (EMG) if a nerve root problem is suspected
Treatments
Care for cervical osteoarthritis is multidisciplinary and tailored to symptom severity:
- Pain relievers and anti-inflammatories (NSAIDs): first-line medication to ease pain and reduce inflammation during flare-ups
- Muscle relaxants: for significant associated muscle spasms
- Physical therapy: a neck rehab program to strengthen surrounding muscles, improve posture, and restore mobility
- Soft cervical collar: during acute painful flare-ups, on a doctor’s advice and for a limited time
- Corticosteroid injections: if oral treatments fail, done under imaging guidance
- Surgery: reserved for severe compressive forms with neurological deficits, such as cervical discectomy and fusion
Prevention
Simple lifestyle measures can help prevent or slow cervical osteoarthritis:
- Practice good ergonomics at work: set screen height at eye level, use a supportive chair
- Get regular, appropriate exercise (swimming, yoga, walking) to maintain neck flexibility
- Avoid prolonged forward head posture (phone, tablet use)
- Use an ergonomic pillow suited to your body for good neck support at night
- Maintain a healthy weight to reduce mechanical strain on the spine
- Do gentle daily neck stretches
When should you see a doctor?
See a doctor promptly if neck pain does not go away within a week of self-care, or for arm pain with tingling or numbness, intense dizziness, unusual headaches, or coordination problems. Muscle weakness in the limbs or bladder and bowel problems are a medical emergency.
